Hourly Mood & Medication Chart - This chart is designed for rapidly cycling children. The day is broken into hours and there are places to record mood, energy, rages & their triggers, sleep patterns & disturbances and medication. Parents should fill out this chart.

Rainbow Chart - This chart is designed to track 3 emotions (sad to happy, angry to satisfied and frustrated to peaceful), energy level (tired to energized) and cognition (confused to sharp minded). The child rates his own levels from 1 to 10 on a rainbow-colored chart three times daily. There is room under each rainbow chart for details such as medications taken, sleep disturbances or school experiences.

Boys & Girls Mood Charts - These charts come in two types: daily charting and monthly charting. Daily charts are one day per sheet to be kept journal-style. The information off the daily sheets can be transferred concisely to the monthly sheet (one month per sheet). Because I have used graphics of faces showing emotion, there is a boy set and a girl set. Either the child or the parent may keep these charts.

Thermometer Chart - This chart is designed to be used by the child in school. The child places a post-it note on the thermometer to indicate his stress level. Calming techniques are listed on the right side of the chart to encourage self-directed coping skills.
